What is alpha-1 antitrypsin deficiency (AAT deficiency)?

Alpha-1 antitrypsin deficiency (AAT deficiency, or AATD) is an inherited condition that raises your risk for lung and liver disease. If you have this condition, your body doesn't make enough alpha-1 antitrypsin (AAT).

AAT is made by your liver. It helps protect your lungs from inflammation and irritating substances you might breathe in, such as smoke. If your liver doesn't make enough AAT, your lungs may be more easily damaged from smoking, pollution, or dust from the environment. This can lead to a serious lung condition called chronic obstructive pulmonary disease (COPD). AAT deficiency may also cause a liver disease called cirrhosis. This is more common in children who have AAT deficiency.

What causes alpha-1 antitrypsin deficiency (AAT deficiency)?

AAT deficiency is a genetic disorder. That means it's caused by changes in your genes, which may also be called gene variants or mutations.

Your genes carry information that controls what you look like and how your body works. AAT deficiency is caused by changes in the SERPINA1 gene, which carries instructions for making the AAT protein. These gene changes are inherited from your parents, so AAT deficiency tends to run in families:

  • If you have two mutated copies of the gene, it means you have a condition called AAT deficiency. People with this disorder have a higher risk of getting lung disease or liver damage before the age of 45.
  • If you have one mutated copy of the gene, you are a carrier of AAT deficiency. In these cases, this means you are at slightly higher risk of developing lung disease, especially if you have other risk factors, such as being a smoker. You could still pass the mutated gene on to your children.

There are a few gene changes that cause AAT deficiency. These gene changes can:

  • Decrease the amount of AAT protein your liver makes.
  • Prevent your liver from making any AAT.
  • Affect the shape of the AAT protein so that it can't move out of your liver to protect your lungs. Over time, AAT builds up in your liver and causes damage.
What are the symptoms of alpha-1 antitrypsin deficiency (AAT deficiency)?

Some people who have AAT deficiency do not have any symptoms. For those who do, symptoms usually appear in people between 20 and 50 years old. These symptoms may include:

  • Wheezing
  • Shortness of breath, especially after exercise
  • Chronic cough with phlegm (mucus)
  • Repeated respiratory infections such as colds and the flu
  • Chest pain
  • Fatigue
  • Faster-than-normal heartbeat when you stand up
  • Vision problems
  • Losing weight without trying

Some people who have AAT deficiency may have liver damage. Signs of liver damage include jaundice (a condition that causes your skin and eyes to turn yellow) and swelling in your legs.

Rarely, AAT deficiency can cause skin problems, such as painful lumps or patches.

How is alpha-1 antitrypsin deficiency (AAT deficiency) diagnosed?

Your health care provider may test you for AAT deficiency if you have:

  • Symptoms of AAT deficiency
  • A condition that could be related to AAT deficiency
  • Relatives who have AAT deficiency
  • Relatives who have a lung or liver disease that could be related to AAT deficiency

AAT deficiency in babies often affects the liver. Your baby may need AAT testing if he or she has signs of liver disease such as jaundice or abnormal liver enzyme tests.

A blood test can check the level of AAT protein in your blood. If the level is lower than normal, it is likely that you have AAT deficiency.

A genetic test is the most certain way to check for AAT deficiency and should be done to confirm the results of the blood test. There are two types of genetic tests:

  • A genotype test looks for the more common types of gene changes that can cause AAT deficiency.
  • A phenotype test checks for changes in the AAT protein that change how it would normally work.

If your lungs are affected, you may also have lung function tests to see how well your lungs are working.

What are the treatments for alpha-1 antitrypsin deficiency (AAT deficiency)?

There is no cure for AAT deficiency, but there are treatments to help with the symptoms and slow the lung damage it causes. Treatment options may include:

  • Inhaled medicines to help you breathe better.
  • Pulmonary rehabilitation.
  • Oxygen therapy.
  • Augmentation therapy, which is a lifelong treatment. It raises the levels of the AAT protein in your lungs, using ATT protein taken from the blood of donors. It helps slow down lung damage. It cannot prevent liver damage.
  • Lung surgery or a lung transplant, if your lungs are severely damaged.
  • Liver transplant, if your liver is severely damaged.

To help prevent or delay lung damage, it is important to quit smoking (if you smoke) and avoid secondhand smoke, dust, and air pollution. Ask your provider If you need to stop drinking alcohol.

ALKAL-ANTISEPTINE
   A pleasantly aromatic liquid having an alkaline reaction and serving as an efficient antiseptic and germicide although not pois onous to the human subject. It differs from Antiseptine, q. v. in that it contains glycerin and is distinctly alkaline. Its chief active constituents are Thymol, Eucalyptol, Menthol, Sodium Borate and Sodium Benzoate. Internally it may be given in teaspoonful doses as a gastric and intestinal antiseptic with antacid properties, but it is much more commonly used locally 3

AMYL NITRITE
   Antispasmodic, reducing arterial tension and so causing momen tary acceleration of the heart beat. Used chiefly by inhalation to relax spasm, notably in asthma, in whooping cough. in spasmodic croup, in epileptic seizures, in tetanus and strychnine poisoning and in spasmodic dysmenorrhea. It is a valuable agent also in angina pec toris and in sudden heart failure, but must be used always intelli gently and with due caution remembering that it is a depressant, not a stimulant. Dose. usually 0.13 3

BONESET (Eupatorium)TAMAU2
   The leaves and flowering tops of EUPATORIU M PERFOLIATUM, Lin. Tonic, diaphoretic, febrifuge. in large doses laxative and nauseant or emetic. The remedy deserves more attention than it has received, its range of use being essentially the same as that of Matricaria.3

CINCHONIDINE
   Alkaloid from CINCHONA BARK, having the general properties of Quinine (q. v.) but not so liable to produce ringing in the ears and vertigo as that alkaloid. Dose of the sulphate as tonic, 0.015 to 0.13 Grm. (1/2 to 2 grs.) three times a day; as antiperiodic, 0.6 to 1.3 Grm. (10 to 20 grs.) during the six hours preceding the expected paroxysm.3

COD LIVER OIL
   The oil obtained from the fresh livers of GADU8 MoRRHUA, Lin. Readily digested and having a peculiar power of improving nutrition. It is believed that this property resides in certain constituents of the oil not of a fatty nature and accordingly some of the preparations contain the non-fatty portions only of the oil. Ordinary dose for an adult, 15 c. c. (half a fluidounce), three times a day.3

COPPER ARSENITE
   Intestinal antiseptic; prescribed in diarrhea either iodiop athic or that attending typhoid fever. Dose, 0.00006 to 0.0025 Grm. (1-1000 to 1-25 gr.).3

COTTON-ROOT
   The bark of the root of GOSSYPIUM HERBACEUM, Lin. Emmena gogue, oxytocic, abortifacient. Its action resembles that of ergot.3

DUBOISIADUBOI
   The leaves of DUBOISIA MYOPOROIDES, R. Br. Resembles Bella donna in its medicinal properties, but more distinctly hypnotic and calmative. The extract, also the alkaloid derived from the leaves, is used to produce mydriasis, its action being more prompt than that of atropine, and the effect passing off more quickly.3

ECHINACEA
   The root of BRAUNERIA PALLIDA, (Nutt.) Brit. [Echinacea angustifolia, DC.]. Antiseptic, vulnerary, alterative; has a pungency like that of pellitory. Locally applied has a powerful aphrodisiac action.3

CATARRH, NASAL (See also Coryza)
   Chronic Nasal Catarrh in our climate is an intractable affection. In mild cases Alkal-Antiseptine, well diluted with water and ap plied twice daily by aid of the little douche that accompanies it, is all the treatment needed. Application of Iodosyl once daily has a most happy effect. Tonics are often indicated. Atrophic Nasal Catarrh calls for use of more active antiseptics and a stimulating treatment, silver nitrate, iodine and hydrogen peroxide being the most useful remediés.3

CHOLERA, ASIATIC
   As prophylactic measures, avoid all uncooked foods, especiaHy water that has not been boiled; sulphuric acid lemonade. GASTRIC ULCER
   Nourish patient by enemas so that stomach may have complete rest. Internally, silver nitrate and resorcin with codeine or morph ine, antacids, especially sodium bicarbonate with bismuth subcar bonate; counter-irritation by tincture iodine or mustard leaves; antiemetics as needed; for hematemesis, emulsion of oil turpentine, antipyrin, alum or other styptic; suprarenal extract. Bowels must be regulated by enemata.3

GOITRE
   In simple Goitre (Bronchocele), ointment of mercuric iodide or tincture iodine externally; creosotide or potassium iodide and extract of thymus gland internally. In Exophthalmic Goitre, belladonna, strophanthus. ergot and sparteine are among the remedies that have been found serviceable. Extract of suprarenal capsules promises good results.3

HEADACHE
   Seek the cause and govern treatment accordingly. Distinguish articularly congestive from anemic headaches. In the former ergot and vascular sedatives are useful, in the latter stimulating remedies are indicated. See that excretions are normal, that the eyes are not under strain from errors of refraction. that hygenic conditions are good and that the patient takes suflicient exercise and recreation.3

HEART DISEASE
   In organic diseases of the heart. remedies must be adapted to symptoms presented. PRURITUS
   In absence of local cause, general Pruritus calls for tonic treat ment, particularly for use of arsenic. Give attention to stomach, bowels and kidneys.' The local remedies that give most relief are lotions of borax or of sodium bicarbonate or hyposulphite to which carbolic acid may be advantageously added (one or two drachms to the pint). Menthol. resorcin, thymol. boric acid and ichthyol are also useful. Alkal Antiseptine, diluted with from four to eight parts of water is an excellent remedy.3

SCABIES
   Apply parasiticideointments; sulphur, naphthol, mercuric chlo ridaammoniated mercury, potassium a1senite (Fowler's solution) are among the most eflicient, and sulphur in some form is that commonly selected.3
